Most of us have had the unfortunate and memorable experience of suffering through the flu. For others, it can be a life or death situation. It is estimated that flu associated deaths range from 3,000 to 48,000 a year in the United States. In light of this, researchers at the University at Buffalo are asking for your help with a National Institutes of Health funded survey that will help identify effective strategies to control flu epidemics. Your participation is very valuable and may potentially save lives. The survey is intended to be completed by all members of your household. For the purpose of this survey, a household is defined as all people living in the same dwelling as yourself (e.g. family members or apartment-mates). Due to federal regulations regarding minors, we ask that you complete the survey on behalf of any of your minor children (ages 0-17). Children 13 to 17 may complete the survey on their own with parent/guardian permission. At the conclusion of the survey in August, each participant that fully completes the survey will receive a $30 cash award. Parents/guardians will also receive an additional $30 for completed surveys submitted on behalf of each minor child in your household. The survey is made up of a one-time initial survey which you will complete on the survey website during the registration process. You will then be asked to install the survey app to your smartphone and complete a 5 minute survey once a week for 6 weeks beginning July 18, 2016, Monday through August 22, 2016, Monday. The initial survey will ask questions regarding your face-to-face contacts and basic demographic information. The weekly survey will ask questions regarding flu-like symptoms, protective actions, and frequently visited places (location data based on your phone settings). We highly recommend you use your primary phone to complete the weekly survey. If you do not own a smartphone, we thank you for your time and encourage you to explore our website for more information on how to protect yourself and others from the flu virus. Key Features: 1. This survey app has integrated Apple's Researchkit Framework, which allows you to participate in research studies easily through your iPhone. 2. The feature Flu Risk Map helps participants to learn areas where flu symptomatic cases occurred the day before. 3. This project has been approved by the Institutional Review Board at the University at Buffalo. 4. This project is funded by the National Institutes of Health. Continued use of GPS running in the background can dramatically decrease battery life. A Few Things to Keep in Mind: 1.To earn points (and extra cash), you must participate in both aspects of the survey each week (30 points for completing your survey and 70 points for keeping your location services on, for a total of 100 potential points). 2.Be sure to keep your notifications on so we can send a reminder each week to complete your survey. This way, you increase your chances of earning as much cash as possible! Please note that parents or guardians completing a survey on behalf of any minor children will only be eligible to receive the base $30 cash award for each minor child s full participation as we are unable to collect location points for the minor children.
